The following table presents a chronology for the first quarter of 2016, showing all known ‘fatal’ U.S. civil aviation accidents, as well as a few selected non-fatal accidents and incidents. Data shows date (weekends are underlined), location, number of fatalities, and a brief synopsis. Click on the blue dates to see additional info; click on red dates to see an aiREFORM Post/Analysis about the incident. For all accidents, further records are quickly found by searching for the accident date within NTSB’s Monthly Aviation Accident Database. Those accidents within the most recent two weeks may not be listed yet at NTSB; they are listed from news reports and from the quickly updated databases at Aviation-Safety Network. The following color-codes are added to aid in tracking accident trends. Midair includes NMAC’s. EMS/Commercial/Ag includes flights in-service or related to that flight function. (~1215)A Cessna 172 Skyhawk crashes during an instructional flight to Runway 5 at Yeager Airport [KCRW]. It comes to rest inverted and a small fire erupts. The fire is quickly put out and the two occupants are taken to the hospital. The female flight instructor dies of her injuries, the male student pilot was reportedly in the ICU.[article]

(~0000)An Aerospatiale AS350 helicopter, on a HEMS mission operated by Haynes Life Flight LLC, crashes 3-minutes after taking off with an auto crash victim, reported to be unconscious and with a broken leg. A ‘missing’ report is called in at 12:17am, but the search is delayed by fog and low ceilings. The wreckage is found 7-hrs later, at 7:02am.[article]

(~1550)A North American AT-6 Texan departs Pearson Field [KVUO] in Vancouver, and flies to the Astoria area. The pilot is carrying a recent widow, with the intent of letting her disperse the ashes of her late husband over the mouth of the Columbia River. Witnesses see the aircraft crash into the river. The bodies are located two days later.[article]

(~1202)A former ‘Bachelor’ contestant, a 41-yr-old woman and student pilot, crashes while taking off in an Airborne XT-912 weight-shift-control ultralight. The aircraft erupts in flames upon impacting a curb between [KHHR] and a freeway.[article][NTSB]

Rostov-on-Don, Russia (~0341)FlyDubai Flight #981 crashes on the airport at Rostov-on-Don [URRR], killing all 62 persons on board. The budget carrier flew a Boeing 737 from Dubai more than four hours, then executed an ILS Runway 22 missed approach in poor weather. The crew then flew two delay loops before entering a holding pattern to wait out the weather. After nine loops and nearly a full hour in the holding pattern, the flight made a second attempt on the ILS Runway 22 approach. Inexplicably, a second missed approach was initiated at approx. 3.5-miles from the runway; the aircraft then dropped from the sky, disintegrating on impact with the airport surface. The weather, pilot fatigue, and possible terrorism have all been suggested.[article][A-S.N]

The pilot, a businessman, had obtained his pilot’s license last fall. Online weather is not posted for Alpine, but the Idaho Falls METAR showed marginal flight conditions with a low ceiling. (~2325)A United Airlines feeder Embraer ERJ-145, operating as Trans States Airlines Flight #4615 from O’Hare, reportedly has multiple wingtip strikes on the runway during a crosswing landing attempt. The flight goes around, and successfully lands on the second attempt. The pilot and crew make no comments to the passengers. (~2305)Engine troubles are reported from a Piper PA28 Archer on a training flight from Fitchburg, MA [KFIT] to [[KFRG]]. The flight instructor reportedly takes over and ditches in cold water near Poquott. The pilot and two passengers are rescued by men who hear screams and miraculously locate three kayaks, which they use to tow the survivors ashore. One other passenger, a college student from Queens, is not found and presumed dead.[article]

(~1900)A Beech BE36 flying from Daytona Beach [KDAB] to Lexington, NC [KEXX] iin fair weather turns off course in southern NC and then crashes in a wooded area, killing the 68-yr-old male pilot. News reports indicate the pilot, who had rouglhy 20,000 hours of experience, had reported a loss of engine power to ATC. The NTSB Preliminary report suggests fuel starvation was a likely cause.[article][NTSB]

The pilot is unable to land at the Contemplation Circle monument due to pedestrians, and instead makes a forced landing in water 20-feet from the shore. Flightaware indcates the aircraft, based at KSTS, had a flightplan to depart Palm Springs at 5:02pm for a return home. The NTSB preliminary indicates the flight departed earlier, accepted VFR flight following, and began an IFR flight 46-miles from destination. ATC had cleared the flight for an ILS approach to Runway 32, and the tower had already cleared the flight to land.[article][

ATC offers altitudes and other assistance and is ready to clear the flight on to KTUS when the flight simply drops off radar and crashes, leaving a linear debris field said to be a mile long. Weather appears not to have been a factor, though the ceiling included a 2,800-ft broken layer.[A-S.N][NTSB]

(~1253)A Cessna 208 (single-prop) operated by Wright Air Service on a commuter flight out of Fairbanks crashes roughly ten miles from its destination airport [PAKP], causing serious injuries to the seven passengers and pilot. Passengers interviewed by TV station KTVA report calm winds but the aircraft iced up: “…it started getting bad, the wind shield was icy and [we] could barely see.” She also explained that she was dozing off, in and out of sleep during part of the flight: “I remember waking up, and we were getting lower and lower, and all of a sudden we turned and hit the mountain.”[article]
