The following table presents a chronology for the second quarter of 2016, showing all known ‘fatal’ U.S. civil aviation accidents, as well as a few selected non-fatal accidents and incidents. Data shows date (weekends are underlined), location, number of fatalities, and a brief synopsis. For all accidents, further records are quickly found by searching for the accident date within NTSB’s Monthly Aviation Accident Database. Those accidents within the most recent two weeks may not be listed yet at NTSB; they are listed from news reports and from the quickly updated databases at Aviation-Safety Network. The following color-codes are added to aid in tracking accident trends. Midair includes NMAC’s. EMS/Commercial/Ag includes flights in-service or related to that flight function. The aircraft continues on its heading and crashes near Mount Wilson in the Angeles National Forest, noth of Altadena. The last radar target posted online was at 8:29am; the overdue aircraft report to the LA County Sheriff happened shortly after 9:10am.[article][FlightAware]

(~1647)A Pitts S2 crashes on airport during a two-aircraft performance at an airshow. A crossover had been performed and the Pitts pulled into a loop but impacted terrain prior to coming out of the loop, killing the pilot. A line from an AJC article read: “There was a large crowd that was still here watching the air show when this occurred, so we know this is going to be very traumatic to a lot of people who witnessed this unfortunate event.”[article][NTSB]

(~1413)A Cessna 208B Grand Caravan carrying 17 skydivers loses power while climbing through 1,000-ft, shortly after taking off fromm the Lodi Municipal Airport. A forced landing follows. It flies under powerlines then impacts a pickup truck before crashing in a vineyard and inverting. A 2-minute video from a helmet-camera of one of the skydivers covers the crash and escape events. A line in the LATimes article reads: “The plane is owned by Nevada-based Flanagan Enterprises Inc., according to FAA records. The company’s president listed in Nevada public records, Ian Flanagan of Alberta, Canada, could not be reached for comment.”[article][NTSB]

(~1655)A mechanic who is also a flight instructor is asked to complete a maintenance flight in a Beech Bonanza BE35. He elects to use the same flight to provide instruction, departing out of the Glendale Municipal Airport [KGEU]. Witnesses report seeing the flight descending over a parking lot “low, slow, and quiet.” The flight crashes onto a greenbelt area of a senior community. The mechanic was killed and the student was seriously injured.[article]

(~1408)A 39-yr-old male pilot is hospitalized in serious condition after his AS350 helicopter, operated by ERA Helicopters, impacts the Norris Glacier in ‘flat light’ conditions. The helicopter reportedly is used to offer local ‘flightseeing’ tourism as well as support to dog camps. Weather conditions were reportedly ‘unfavorable but not abnormal’. PAJN METAR included winds ESE 17 visibility 8 light rain& clouds FEW013 BKN039 OVC050, so it appears operations at the glacier were likely in low light, low ceiling, and low visibility. [article]

The flight examiner takes over and attempts a left turn to return and land, but the flight impacts terrain a third of a mile from the runway end. A post-crash fire erupts. The flight examiner is killed and the ATP applicant (a National Guard pilot with 950 total hours) is seriously injured.[article][NTSB]

(~1000)An MD369 helicopter operated by Rotor Blade LLC in South Carolina crashes while doing support work for powerline maintenance. Witnesses report seeing the helicopter maneuver at low altitude, to face into the wind, then hearing a change in the engine sound just prior to seeing the helicopter drop and impact trees. The 61-yr-old male pilot, from Madison, was killed in the crash.[article][NTSB]

(~1700)An Extra EA300 crashes into the desert east of I-15, killing both occupants. The aircraft was registered with Vegas Extreme Adventures, a business based in Henderson that sells aerobatic flights. The crash location, south of Las Vegas, is used for selling variopus aerial thrills; in early 2015, an 11-yr-old boy and a 55-yr-old ‘instructor’ were killed in the same area in a fatal hang glider crash.[article]
