The following table presents a chronology for the second quarter of 2014, showing all known ‘fatal’ U.S. aviation accidents, as well as some non-fatal accidents. Data shows date (weekends are underlined), location, number of fatalities, and a brief synopsis. For all accidents, further records are quickly found by searching for the accident date within NTSB’s Monthly Aviation Accident Database. Those accidents within the most recent two weeks may not be listed yet at NTSB; they are listed from news reports and from the quickly updated databases at Aviation-Safety Network. (~10:30AM) A Cessna Sklyane C182 experiences engine trouble and diverts but fails to reach the new airport. The sole occupant dies in the accident. He was an aviation attorney and a decorated Army officer in his late sixties. [article]

Ottawa, IL

(~8:30PM) A powered parachute takes off from an airport southwest of Chicago, commonly used for skydiving. A nearby neighbor becomes concerned when she sees it flying low near Dayton Dam, on the Fox River. The aircraft loses power and crashes in floodwaters downstream from the dam, then jams up onto a log. The 39-yr-old pilot is able to cut himself free, but unable to help free the 50-yr old male passenger before trying to go for help. The pilot leaves, makes it to shore, and notifies rescuers at around 10PM. The high flood waters prevent a rescue that night. The passenger’s body is retrieved the next day. [article]

(~3:55PM) A Christen Husky hangared at the Tampa Executive Airport [KVDF] takes off, then almost immediately nose-dives into the runway, killing the 54-yr-old male pilot. [article]

(~2:20PM) A kit-built Advanced Soaring Concepts Spirit glider is seen by multiple witnesses, flying low to the horizon in gusty conditions. The 60-yr-old male pilot dies when the aircraft impacts the terrain. [article]

(~11:30AM) A kit-built BD-4, constructed in 1999, crashes into a cornfield east of the airport, shortly after takeoff. It catches fire and the owner/builder is found deceased. [article]

Livermore, CA

(~4:55PM) A pilot departs the Livermore, CA airport [KLVK] in a kit-built Loehle Mustang (a 3/4-scale P51 Mustang made from wood and fabric). A fire is observed and the pilot jumps out of the aircraft to the northeast of the airport, falling 1,000′ or more to an instant death. The aircraft crashes near the freeway and starts a grass fire that is quickly extinguished. One report indicated there may have been a second victim on board. [article]

St. Cloud, MN

(~8:26PM) An RV-6 (kit-built) departs St. Cloud Regional Airport [KSTC] and the pilot tells the tower he is going out to do some aerial photography. About a half hour later, while over Sauk Rapids and roughly 6-miles NW of the airport, the RV-6 suddenly plunges into a house on a cul-de-sac. News reports show multiple witnesses at nearby homes reported seeing a commercial airliner at a surprisingly low altitude, just before seeing the diving small plane. An Allegiant Airbus A319 had a flight path and descent profile consistent with the witness reports, and landed at the St. Cloud Airport roughly three minutes later. Two fatalities are found, and the one person in the house escaped without injury. [article]

(~afternoon) A Mooney M20 based in South Carolina flies toward California on a long trip. The 47-yr-old male pilot has a 15-yr-old family friend, a boy who loves to hike and hunt and who had just finished ninth grade. They get to Las Vegas, then fly to Page, AZ for a fuel stop. Taking off again, they head east to sightsee the southwest terrrain. The aircraft disappears and the wreckage is found two days later in Peach Canyon. [article]

San Juan Mountains, UT

(~morning) Two pilots are each flying their own plane on a camping trip. They both depart Durango, CO and head west, planning to meet again when they stop for fuel at Brigham City, UT. Radio contact is eventually lost in the mountains, but that caused no concern until the accident flight failed to arrive. A search eventually finds the lost aircraft in a ravine. [article]

(~4:30PM) A Piper Malibu PA46 encounters thunderstorms while over west Texas, midway into a flight from Aspen [KASE] to a small Houston-area airport [11R]. The flight takes an unusual turn toward the south and southwest, passes through the center of a strong storm cell, and crashes on grazing land. [article] – [flightaware]

(~2:21PM) A Westwind II jet departs Runway 18R at [KHSV] in fine weather conditions, and ends up crashed in a cotton field to the right near the departure end of the runway. The deceased men are ages 57, 60 and 67. Reports state that two of the men were corporate pilots for the firm in Birmingham, AL that owned the plane, and they were doing practice approaches. (~5:30PM) A Grumman G21 twin-engine Goose is seen in a spin before it crashes into a parking lot at Lost Pass on the Montana-Idaho border. Witnesses say the weather had been changing all day, from rain to heavy snow to clear skies. It was snowing at the time of the accident. The aircraft quickly burst into flames. [article]

(~9:00AM) A Cessna LC41 takes off from Islip Airport [KISP] for a short flight to Republic Airport [KFRG], also on Long Island. The flight crashes into an above-ground pool between houses, then explodes in fire. Weather may have contributed to the severity as the flight took off with a reported ceiling of 700′ overcast. (~9:45AM) Witnesses hear a pop then see a Grumman AA5 crash 500 yards offshore, near the North Jetty, likely killing both the pilot and the passenger. One body is recovered, but the second aircraft occupant remains missing, and is presumed dead. [article]

(~7:45AM) A group of ten from New York is on a ride in a hot air balloon operated by US Hot Air Balloon Team. While landing in a field near Philadelphia, the 47-yr-old balloon pilot reaches out to pull a rope, but he falls from the basket, and is dragged under the landing basket. [article]

(~2:15PM) A skydiving operation flies a Cessna Skylane C182 out of the Festus, MO airport. During a routine flight, six skydivers jump, but the sixth made a loud ‘thud’ when he hit the rear of the plane (he was not injured). The elevator was disabled, and the aircraft was losing speed and stuck in a descent. The 21-yr-old pilot jumps at around 1,500-ft and lands safely with his parachute, while the plane circles to a crash in a cornfield. [article]

(~1:20PM) A 65-yr-old male dies when his replica P51 Mustang crashes in a cornfield. The crash location was near the airport where he hangared the airplane. News reports indicate the replica Mustang was built by others, not by the pilot. [article]

(~8:25AM) A Piper Malibu PA46 takes off from White Plains [KHPN] into a low ceiling and clouds, then crashes within minutes, in a residential area roughly 3-miles north of the airport. The pilot and owner was Richard Rockefeller, age 64. [article]

(~8:55PM) A witness calls in that a small plane crashed near Echo Bay Airport, at the north end of the Lake Mead National Recreation Area. (~2:40PM) A Bell 206 helicopter crash was reported by crews on an offshore rig, approximately 60-miles south of Terrebonne Bay. A rescue dinghy failed to retrieve the two on board. The helicopter is missing and submerged in the waters. [article] – [article]

(~9:55PM) A Cessna Skyhawk C172 operated by a local flight school loses engine power shortly after takeoff on an instructional flight. An attempt was made to return to the airport, but the Cessna stalled and erupted in flames when it crashed. The flight instructor and student pilot were both 22-yr-olds. [article]

(~8:30PM) A Liberty XL-2 strikes a powerline and crashes into a backyard during a flight by a prospective buyer. The aircraft was owned by a flight instruction business, and the 64-yr-old prospective buyer went for a flight with the 47-yr-old business owner. Both men died in the crash. [article]

(~11:10AM) A Grumman AA-1 loses control, does a series of barrel rolls, and crashes into the dry lake bed near Adelanto, CA. The 47-yr-old pilot was a helicopter veteran with the LA Fire Department. His passenger, a juvenile, is hospitalized with serious injuries. [article] UPDATE, 6/16/2014: — the passenger, age 8, died in the hospital. Authorities are saying there may have been a mechanical failure that caused the flight to nose-dive into the ground. [article]

(~11:30AM) An experimental kit-built Schrieber P-70, when it crashes near the Great Bend Airport. The 49-yr-old pilot had a multi-generational family farm on lands adjacent to the airport. His passenger was a 24-yr-old male. [article]

(~1:00AM) An MD-500 helicopter operated by the Mesa Police Department lost power and made a crash landing into a field. Two were on board, and neither was injured. [article]

(~night) A Cessna Skyhawk C172 clips a patio awning while preparing to land, then crashes into a backyard, just feet away from a family gathering. Three occupants stepped out uninjured. The aircraft reportedly is owned by American Aviation, the same outfit based at the Page airport that had a fatal accident a month earlier, on May 10th. [article]

(~11:20AM) A kitbuilt Lancair IV is sold to a new owner in Bend, OR. He flies it to Duluth, then takes off for another long leg to Goose Bay in eastern Canada, apparently on a ferry flight to his home in Germany. The aircraft crashes into Lake Superior not long after departing. Search efforts are delayed by fog and low visibility. The wreckage was eventually located at a depth of 140-feet, and a submersible confirmed that the deceased pilot was still aboard. [article]

(~7:15PM) A Cessna Skyhawk C172 impacts a tree while on approach to a private airstrip near Laytonville, CA. The right wing is sheared and the aircraft comes to rest on a gravel bar in the river, killing the pilot, who was the lone occupant. [article]

(~4:20PM) A USMC Harrier jet, based out of Yuma, crashes into a neighborhood, bursting into flames and damaging three houses. The pilot safely ejected, and nobody in the houses was hurt. [article]

(~1:40PM) A parachute operator parks a de Havilland DHC-6 Twin Otter at the end of a shade tunnel, but leaves the engine running while loading jumpers. An employee, age 24, walks out to talk to a flight crew member. She steps into the spinning prop and dies in the hospital. [article]

(~12:30PM) A Yakovlev YAK-55 performing aerobatic maneuvers at the Stevens Point Air Show fails to come out of a slow, wobbling dive, and crashes in woods near the airport. The performer, a 47-yr-old male, is killed, and the air show is immediately ended. [article]

(~9:40PM) A Gulfstream IV-SP crashes past the departure end of Runway 11, when taking off on a return flight to Atlantic City, NJ. News stories are not clear if it caught fire before or after the crash, but when the aircraft stopped near the Shawsheen River, the flames burned sixty feet high.

Family members tell reporters that the pilot was taking people for short scenic flights, and the last known departure was past midnight, at around 12:30AM. (~7:37PM) A kit-built experimental Glasair is damaged when it lands gear-up. News reports have not yet clarified whether this is an emergency landing with a gear failure, or an inadvertent gear-up landing. A point of pride for seasoned controllers is to remain alert and ‘save’ a gear-up landing by seeing it and warning the pilot before it is too late to recover. The pilot was not injured. [article]

(~5:30PM) A kit-built amphibious plane attempts to take off on Banks Lake, and a video is recorded by a boater showing the aircraft bouncing on wave tops. A hard bounce flipped the aircraft and trapped the 60-yr-old pilot underwater. Nearby boaters were unable to rescue him in time. [article]

(~4:00PM) A Beechcraft BE36 Bonanza crashes while approaching to land at the Whiskey Creek Airport [94E]. Weather was likely a factor, as there were strong gusting winds, with a crosswind gusting to 28kts at a nearby weather reporting station. The pilot and three students from the Aldo Leopold Charter High School are killed in the accident. [article]

(~11:30AM) A Grumman American AA-1B takes off from the Wilmington, NC Airport to fly to Hartsville, SC. The flight never arrives. A call is made two days later, reporting the non-arrival. The wreckage is located on Monday, three days after the crash, after the ELT signal is detected. The location was 3.5 miles southeast of Chester, which is 60-miles further west of the intended destination. [article]

(~10:10AM) A Robinson R22 helicopter rented from Channel Island Helicopters (based in Oxnard) impacts a powerline near the Santa Paula airport, then crashes into the dry wash below. The 42-yr-old male pilot is killed. News reports note that a similar fatal powerline impact happened at essentially the same location, just a few years ago. [article]

(~morning) A news helicopter operated by WHDH-TV takes off, then immediately develops engine problems. The flight climbs to twenty feet in the air, then veers right, crashes downward to a hard landing, breaking the tail off. The Bell 206 is substantially damaged, but the two occupants escape injury. (~1:30PM) Allegiant flight #142 to Wichita is evacuated away from the gates after a flight attendant smelled smoke or a chemical odor near the rear of the MD80 during taxi for takeoff. Six minor injuries were reported and a replacement flight departed just after 5PM. [article]

(~4:00PM) A new Eurocopter AS350 operated by Papillon (air tourism) was repositioning in the bottom of the Grand Canyon near [1G4] when it rolled over onto its side. The pilot was killed. At least one commenter to a news article mentioned strong winds. The nearest reported aviation weather station is more than forty miles away. [article] UPDATE 5/27/2014: The NTSB Preliminary Report included this: “…Witnesses reported that the pilot landed and was planning on exiting the helicopter to perform a “fluid level check.” After landing, the pilot exited the running helicopter; shortly thereafter the helicopter became airborne without the pilot at the flight controls. The helicopter subsequently impacted the ground and rolled over. The pilot was struck by one or more of the main rotor blades and was fatally injured….” PRELIMINARY

(~6:18PM) An airshow performer does low-passes then lands at Peachtree-DeKalb with his Korean War Corsair F4U. There is a strong crosswind which reportedly became a quartering tailwind at the time of his landing. He veers off the runway and flips over. No fire erupts and he steps out and walks away. FAA’s ASIAS does not list the runway being used, and it reported the incident as: “Aircraft landed and went off the runway and into the grass.”

(~5:45PM) A Cessna T207 operated by American Aviation (based in Salt Lake City) is carrying six tourists when it loses power, twenty minutes into the flight. The pilot lands a few hundred feet short of the intended runway, and the aircraft flips over in windy conditions. A 79-yr-old woman from France is killed. [article]

(~8:30PM) A Navion G Rangemaster crashes onto a residential property, three miles west of the intended runway while on approach to the Atlantic City Airport. Four were on board and one was killed. Weather at the time was a half mile visibility in fog.

(~7:56PM) The inaugural Mid-Atlantic Balloon Festival is kicked off with thirteen balloons launching for a short Friday evening flight. As they begin landing in a field, one of them hits a powerline and the basket bursts into flames. The 65-yr-old pilot and his two passengers are killed. [article]

(~9:40PM) An FAA controller working departures issues a ‘right’ turn instead of a ‘left’ turn to a United AirbusA320. The error causes two United departures to converge, then take evasive actions in response to TCAS alerts.

(~12:30PM) A Cessna C150 registered with PlaneLogic LLC crashes in rocky terrain 11-miles southwest of the airport. A witness was mountain-biking and reported hearing the engine sputter before the crash. One article noted the registered owner was a flight school (typical for this aircraft type). [article]

(afternoon) An 84-year-old pilot from Huntsville, AL takes off from Cody, WY, in marginal weather, heading for Twin Falls, ID and eventually Seattle. He is flying on a pleasure trip with his 86-yr-old brother from Seattle. The flight, in a 1963 Mooney M20C, disappears. A search is initiated, and the wreckage is found six days later, high up a mountainside in northwest Wyoming. [article]

(~2:05PM) At the Thunder over Solano Airshow, performer Eddie Andreini dies when his Boeing Stearman biplane impacts the runway while performing an inverted low pass to cut a ribbon. A fire breaks out. [article]

(afternoon) A retired Army Major General, age 61, is being towed for a glider flight. Climbing through 100′ above the ground, the towrope disconnects. Witnesses report seeing him try to make a sharp turn. The Aero Tek Zuni glider stalls, then hits a pole. The pilot, who had commanded a fighter wing and the New York Air Guard before retiring in 2009, was killed. [article]

(~2:33PM) An apparent controller error, with a Gulfstream GLF3 landing Runway 19R while a Delta B737 was taking off of Runway 25R. The 737 had been issued a ‘no delay’ clearance, but the controller cancelled that takeoff clearance 39-seconds later. The B737 reportedly had flat tires, likely from aggressive braking, and passengers were offloaded away from the terminal. [article]

(~4:05PM) A restored Hawker Sea Fury and a Cessna C210 both attend the Pacific Coast Dream Machines event at Half Moon Bay, CA. They then depart to return to their home airport, near Ione, CA, but a collision occurs over San Pablo Bay, near Richmond. The C210 crashes into the bay and the pilot is missing; the Sea Fury continues another forty minutes and lands east of Sacramento, at Ione. At least one article indicated the Cessna may have been flying close to shoot photographs. [article]

(~2:57PM) A Bellanca Viking BL17 departs Shreveport [KSHV] and cruises at 8,000′ on a trip to Sweetwater, TX. The flight possibly ran out of fuel and crashed three miles from the destination. The passenger is killed, and the pilot survives. [Flightaware]

(~unknown) An anonymous report was made at 9:30PM, about a crashed ultralight. Searchers failed to locate the debris at night, but did locate a burned out ultralight and deceased pilot the next day. [article]

(~4:16PM) An apparent controller error. The two flights were nose-to-nose at 33,000 feet and established eastbound and westbound on oceanic route R578. The eastbound flight did an abrupt dive in response to a TCAS alert on the flight deck. FAA remained quiet for three weeks. The incident was exposed by a blogging passenger, then confirmed by an FAA employee under anonymity.

United flight #1243, a B737 arriving from KSFO, was cleared to land Runway 29, while an ExpressJet Embraer was cleared for takeoff Runway 4R. The controller cancelled the landing clearance, then issued traffic to both flights. The Embraer reportedly tips its nose downward to stay under the B737, which crossed overhead from right to left. The aircraft landing gear was lowered and a forced landing was made in a field. The aircraft was stopped in a nose-down position, suggesting the field was wet and caused a rapid deceleration.
