The following table presents a chronology for the third quarter of 2014, showing all known ‘fatal’ U.S. aviation accidents, as well as some non-fatal accidents. Data shows date (weekends are underlined), location, number of fatalities, and a brief synopsis. For all accidents, further records are quickly found by searching for the accident date within NTSB’s Monthly Aviation Accident Database. Those accidents within the most recent two weeks may not be listed yet at NTSB; they are listed from news reports and from the quickly updated databases at Aviation-Safety Network. The following color-codes are added to aid in tracking accident trends. EMS/Commercial/Ag includes flights in-service or related to that flight function. (~3:40PM) A homebuilt Kitfox III crashes shortly after taking off from the Frontier Airpark [WN53], west of Granite Falls. The 66-yr-old male pilot survives, but the 80-yr-old male co-pilot is killed. [article]

Alden, NY

(~10:25AM) More than a half dozen pilots are flying out of the Buffalo-Lancaster Airport, offering rides to kids as part of the EAA Young Eagles program. On the second flight, a 59-yr-old male dentist in his kitbuilt Searay (amphibious plane) is carrying a 9-yr-old girl at around 800-ft above the ground, when a Cessna Skyhawk propeller impacts the rear of the Searay.  The 78-yr-old male pilot and 14-yr-old boy passenger in the Cessna both die in the crash. The Searay makes a forced landing into a field, and both the pilot and passenger suffer only minor injuries. [article]

(~5:30PM) A 47-yr-old male pilot is carrying a 13-yr-old boy (family friend) in a kitbuilt gyrocopter. Shortly after takeoff, the gyrocopter blade is seen to fold, and the gyrocopter falls into a canal, killing both occupants. [article]

(~evening) A couple from Casper departs on Saturday in a Piper PA28, intending to fly to Denver. The wreckage was reported the next morning, a quarter-mile south of the runway. The pilot was 52; his female passenger was 54. [article]

(~8:54AM) Families in a suburban neighborhood in Liberty Township, north of Cincinnati, are having breakfast when they hear aerobatics overhead. A kitbuilt Acro Sport II biplane loses power and crashes, creating a fire and damaging two homes. Both onboard are killed, but there are no reported ground injuries or fatalities. [article]

(~5:15PM) The 46-yr-old owner (and a motorcycle racer) of TOBC Racing Team flies his T28 Trojan from Maryland with his 72-yr-old mother. He reportedly makes a very low pass over the race registration area on arrival, does barrel rolls, then crashes. Both are killed in the fiery impact, on the grounds of the Virginia International Raceway. [article]

(~morning) A 69-yr-old pilot rents a Cessna 162 Skycatcher in Riverside County. His plan includes an early morning roundtrip flight planned from Thermal to Anza-Borrego. He crashes, and te wreckage is located the next morning. [article]

(~12:45PM) A pilot from Beaufort County, NC and his passenger are flying a kitbuilt Seawind 3000 amphibious plane to a fly-in in Illinois. He reports a fuel pressure problem, and sets up to land. The aircraft catches fire then crashes north of the airport, killing both men. [article][article2]

(~11:00AM) A Piper L4 Grasshopper takes off from Headland airport but disappears when flying to Eufaula. An oil slick is found on a lake. Two men are missing. (~11:20AM) A Bell 206 helicopter operated by Airwest on a year-long contract supporting the Cochise County Sheriff’s Office loses power and makes a forced landing near houses. The hard impact splays the metal tubing under the helicopter (supporting the landing skids). The pilot and police observer are not injured. [article]

(~1:26PM) A Piper Aerostar departs from its home base at Dallas Executive Airport, but crashes on approach to Austin-Bergstrom International Airport [KAUS] one hour later. Police reported a high-velocity impact. METAR shows it was hot (95°) with winds gusting to 22kts and a scattered cloud layer, thus weather was likely not a factor. [article]

(~3:20PM) A kitbuilt Lancair IV-P flying from Knoxville, TN to Jackson, MS advises ATC that the windscreen has become fouled with oil and they cannot see ahead. ATC advises the blind pilot of his position relative to the airport at Collegedale. The 44-yr-old pilot reports he will not make it. The husband and father of four was killed in the crash. [article]

(~10:50AM) A Cessna 180 takes off and is seen suddenly nosing up then diving into a crash. Numerous aircraft were at the airport, during the airfield’s annual Labor Day ‘‘flour bomb drop,’’ a competition in which participants pass over the runway and drop small sacks of flour over a target. It was not clear if the accident aircraft was participating. The front of the aircraft was crushed, killing both occupants. [article]

(~6:50PM) A 35-yr-old USAF fighter pilot based in Mississippi takes his 28-yr-old brother for a flight in a kitbuilt Team Tango Foxtrot 4. Just after takeoff from [KABI], the aircraft banks hard left, as if to return to the airport. The brothers die in the crash. A wife and a child, both seated in the rear, survive with injuries. (~11:50AM) A real estate lawyer takes off from the Centennial Airport [KAPA] in a PA46 Malibu. After a 28-mile hop, he crashes and ends up inverted while attempting to land at his home airpark [KEIK]. The pilot, his girlfriend, and three sons are all killed. [article] – [article]

(~3:10PM) A Sirrus SR22 based at Waukesha [KUES] north of Chicago makes a normal climbout and levels off at FL210 direct to Manassas, VA [KHEF]. At some point the pilot becomes nonresponsive and violates restricted airspace. F-16 fighters shadow the flight and see that the pilot appears slumped over. The flight passes over Manassas at 13,000′ and continues southeastbound before fuel exhaustion and a crash into the Atlantic. [article]

(~evening) A pilot in his 50’s buys a kitbuilt P51 replica and hangars it at the local airport. In what may have been his first flight, he takes off from [KCLS] and makes it across the freeway before crashing into a parking lot. Multiple vehicles are destroyed in the ensuing fire. [article]

(~9:58PM) Four students from Case Western Reserve University took off in a flying club airplane from Cuyahoga County Airport [KCGF] and asked ATC to return and land, noting lack of power. Approval was issued, and the controller then watched the crash when it started its turn back to the airport. The Cessna Skyhawk impacted a guy wire, a telephone pole and a chainlink fence, then came to rest and a post-impact fire started. All four were killed. [article]

(~5:30PM) A Mooney M20L bounces while landing at Montgomery Field [KMYF], then clips the roof of a Target and hits a light pole before coming to rest in a Costco parking lot. A fire erupts. Both the pilot and passenger are hospitalized, and the 80-yr-old female passenger later dies. One pilot notes that go-arounds for the particular runway include climbs to 500 feet then a left turn into the pattern, and a lack of airspeed might cause a stall near the Costco. [article]

(~2:45PM) A Piper Archer PA28 has engine problems and the pilot radios in the blind that they will make a forced landing. While landing on Caspersen Beach, the plane strikes and kills a 38-yr-old father while also seriously injuring his young daughter. One witness reported there was no engine noise. (~1:20AM) A kitbuilt Lancair IV-P takes off after midnight from Holland, MI intending to land at the Bluffton, OH airport [5G7], where the aircraft registration is based. A couple is killed in the crash. Recorded weather in nearby Findlay, OH shows that both the visibility and ceiling dropped, and mist formed in the early morning hours. [article][FlightAware]

(~9:45AM) A Piper Malibu PA46 crashes into a forest shortly after taking off from a private airstrip. The 62-yr-old male pilot is killed. At least one news article indicated the flight was headed to the EAA AirVenture at Oshkosh, WI. At the nearest weather-reporting airport (1A5, 19N in Macon County, NC) a 300′ overcast burned off at around 10:35AM. [article]

Gossi, Mali (~1:55AM) AIR ALGERIE FLIGHT #5017: A McDonnell-Douglas MD83 departs Ouagadougou, Burkina Faso in the early morning, and crashes forty minutes later. The flight was operated by Swiftair (Spain) for Air Algerie, and was to arrive at the Algiers, Algeria airport at 5:10AM. Weather is considered possible, but the thunderstorms were reported to be scattered and of normal strength. The possibility of terrorism is also being investigated. All 118 were killed. [A-SN]

Penghu Islands, Taiwan (~7:09PM) TRANSASIA FLIGHT #222: An ATR-72 departs Kaohsiung, Taiwan on a scheduled commercial passenger flight. It crashes in heavy weather while attempting to land at the Magong Airport. Ten of the 58 on board survived, while 48 died. There were no casualties on the ground. [A-SN]

(~2:17PM) A Bell B206 helicopter crashes, killing the 40-yr-old male pilot from Beaverton, OR. The helo had been contracted to fly low and air-dry cherries for harvest. The flight does not return and a search is initiated at around 4PM. The wreckage is located in a swampy area, near Lillian. Both the 67-yr-old pilot and his 15-yr-old female passenger are found dead. (~7:35PM) A Cessna Skyhawk C172 crashes onto a ledge of the Virgin River Gorge, and both occupants are killed. Some witnesses reported hearing no engine noise. (~3:30PM) A Cessna Skylane C182 based in Flagstaff and piloted by a 22-yr-old male crashes northwest of Sedona, and is reported by hikers. The passengers included a 23-yr-old male from Prescott, and two females, ages 18 and 19, from Saskatchewan and Germany. The four reportedly met at a Christian camp. [article]

Donetsk, Ukraine (~4:19PM) MALAYSIA FLIGHT #17 SHOT DOWN OVER UKRAINE: FlightAware tracking of a Boeing B777 from Amsterdam to Kuala Lumpur is lost in northwest Ukraine at around 3:12PM local time. At around 4:19PM, debris from the Malaysia flight begins falling over a wide area, to the east of Donetsk. News reports emerge with rebels allegedly posting on Facebook about using a surface-to-air missile to shoot down a Ukrainian cargo plane. After the Malaysia crash hits the news, the Facebook posts disappear, and a widespread media/Public outrage erupts  when international access to the wreckage is blocked. A total of 298 died.

(~5:51PM) A Piper Saratoga PA32 attempts to land on a short cross-island private airstrip during thunderstorms. Control is lost and the aircraft ends up in coastal waters on the Gulf side of the island, west of Fort Myers. Divers search through the night and eventually find the body of the male pilot the next day. FAA’s ASIAS posting claims three crew members, but news articles indicate only the pilot was aboard and only one body was recovered. An email was sent to Tampa FSDO on 7/19/2014, seeking to clarify. [article]

He was a WWII veteran, had been a prisoner of war, and was noted for his unstoppable energy. He was also known to perform his banjo, harmonica and other instruments at retirement homes.
