The following table presents a chronology for the fourth quarter of 2014, showing all known ‘fatal’ U.S. aviation accidents, as well as some non-fatal accidents. Data shows date (weekends are underlined), location, number of fatalities, and a brief synopsis. (~6:50PM) A Bell 206L helicopter, contracted by Airwest for the Cochise County Sheriff’s Office, finishes routine maintenance at the Glendale airport (near Phoenix) and is being flown back to Sierra Vista in the early evening. Weather is poor – cold and with a low overcast and rain. The weather was also deteriorating and, in fact, became gusting winds and then light snow later that night. In the vicinity of Benson, the helicopter crashes, killing both male occupants. The pilot was 48-yrs-old and had retired as a Glendale police officer in 2011; the passenger was a 59-yr-old mechanic. (~4:29AM) After nearly 24-hours of light snow and with the air temperature at minus 4 degrees Fahrenheit, a Cessna 404 Titan (twin prop) operated by Key Lime Air takes off from Centennial Airport [KAPA] on a short hop to Denver International [KDEN] to start off a cargo trip. The aircraft climbed out but lost power, then crashed adjacent to homes. The pilot, a retired 55-yr-old police detective, was killed. [article]

(~3:01PM) A Piper Aztec (PA23) takes off from the Port Clinton, OH airport [KPCW] then pitches left and nosedives into a backyard. The 60-yr-old male pilot is killed. Reportedly, the aircraft was on its first flight after undergoing maintenance, though the type of maintenance has not yet been revealed. [article]

(~10:30AM) A Robinson R22 helicopter, operated by Palm Beach Helicopters, crashes roughly 100-yards north of the runway. The flight instructor dies, and the student is critically injured. [article][article2]

Java Sea, Indonesia (~7:30AM) Indonesia AirAsia Flight 8501, an Airbus A320, departs Subaraya, in East Java, [WARR] planning to fly a route to the northwest over the Java Sea to land at Singapore [WSSS]. Monsoonal storms cause the pilot to request route weather deviations, and contact is then lost from ATC. A search is suspended during the first night due to continuing poor weather. Westminster, MD

(~3:49PM) A single-prop Piper is landing on the runway at the uncontrolled airport in Westminster, Maryland [KDMW], when a Pitts biplane overtakes it and collides from above. The Piper safely completes the landing, but the Pitts ends up in the grass near the runway, with a folded right wing. The Pitts pilot was taken to the hospital, while the couple in the Piper declined medical transport. No injuries were reported. [article]

(~1:30PM) A Piper Seneca (PA34, light twin) from Ontario crashes while flying an instrument approach to the St. Clair County Airport [KPHN], near Port Huron. The weather had been steadily poor for more than an hour, and was 1/4-mile visibility, fog, and a 200-feet ceiling at the time of the accident. The 51-yr-old male pilot died. [article]

(~12:36PM) A Cessna Skyhawk (C172) crashes in a near-vertical impact into hills near the airport in Amador County [KJAQ], killing a 52-yr-old man and his 27-yr-old son. The father had reportedly flown to Gilroy earlier in the day, and was returning with his son. The visibility had been 3-miles or more until around 11:40AM, then dropped to and remained at 1/4-mile from 12:20PM until 2:00PM. [article]

(~8:55AM) A Cessna Skyhawk (C172) operated by American Patrols, Inc. crashes roughly 15-miles northwest of the Midland International Airport [KMAF], killing the 28-yr-old male pilot. The company does low-altitude flights under contract, to monitor pipeline conditions and look for damages or problems. Witnesses say the flight was in a descending left turn before the impact and fire. [article]

(~6:20PM) A Beechcraft V35 Bonanza flies a return flight home, from El Dorado, AR [KELD] to Mesquite, TX [KHQZ]. The pilot arrives in the area around sunset, but a low overcast sky had persisted all day at around 600-feet AGL. The pilot flies many circles at 2,000′ and 3,000′ MSL, heads north, comes back south, and eventually crashes into homes northwest of the Seagoville private airport. Seagoville is approximately ten miles south of [KHQZ]. The pilot reportedly lost contact with controllers while setting up to land at [KHQZ]. [article]

(~2:00PM) A small Piper plane  believed to have been on a local sightseeing flight crashed south of Polson. The pilot, a 33-yr-old male from Alaska, was killed, and the passenger was taken to the hospital in serious condition. [article]

(~11:10AM) A 24-yr-old student pilot flying a solo cross-country trip between Greeley and Platte Valley suffers a loss of power at 600-feet altitude. She makes a hard landing, breaking the tail boom off of the Enstrom 280 helicopter, and incurs a minor wrist injury. The helicopter was operated by Mountain One Helicopters, at Erie, CO. (~10:50AM) An Embraer Phenom 100 (very small jet) based in Chapel Hill, NC is seen flying low, gear down, struggling on final to the northwest of the Montgomery County Airpark [KGAI]. It reportedly rotated 90-degrees and fell from the sky. One house was destroyed and three other houses were damaged. Three occupants of the destroyed house were eventually found deceased in a bathroom. [article]

(~2:00PM) A Robinson R44 helicopter registered with Native Range Capture Services in Elko, NV reportedly lost its tail section while flying at 100-200′ feet altitude. It then came down onto the roof of a crafting business. One male occupant was trapped in the burning wreckage; the other male occupant died shortly after being extracted. [article] – [article2]

(~7:00PM) A Piper PA28 is taken for a short flight around the pattern. The 57-yr-old pilot had told his wife he was going to work on his plane and get fuel in Covington. He does not come home. The wreckage is found the next morning in a wooded area, two miles from the airport. [article]

(~9:00AM) A Bellanca Super Viking crashes during takeoff, killing the pilot and sending all three passengers to the hospital with serious injuries. The pilot reportedly was a flight instructor in Salina, KS, and an aircraft mechanic at Kansas State University. [article] – [article2]

(~afternoon) A Cessna 170 departs Spearfish, SD, piloted by the priest for two Catholic churches, in Deadwood and Lead. The priest fails to appear at 4PM mass on Saturday, and a search is started. The wreckage is found late Sunday morning, crashed in steep terrain north of Alzada. [article]

(~noon) A kitbuilt RV-3, said to have been recently purchased, crashes near the airport terminal building. Some witnesses claim the aircraft was near-vertical upon impact. The 29-yr-old male pilot was the president of Francis Aviation. In the previous year, he had purchased the FBO at the Doña Ana County Airport [5T6] in Santa Teresa. Also in the previous year, he won a contract with the state of New Mexico to become the exclusive fixed-base operator and runway manager for the Spaceport America facility, where Virgin Galactic may launch. (~10:15AM) A Piper PA24 Comanche strikes a tree near its destination, the airport in Garberville, CA. The aircraft ends up in the Eel River and the pilot, the chief emergency room doctor at the local hospital, is killed. He was 59-yrs-old and known to regularly commute by small plane from his home in El Dorado County. Photos show no apparent prop damage, suggesting he lost power just before the crash. [article]

(~2:54AM) An Aero Commander AC50 operating on a regular cargo schedule takes off from Runway 31C at Midway [KMDW], planning a quick stop at Chicago Executive [KPWK], then onward to stops at the OSU Airport [KOSU] and Rickenbacker Airport [KLCK] in Columbus, OH. The pilot reports left engine trouble and gets ATC permission to land on Runway 31C. He flies a left pattern and tries to return to land at Midway but crashes a quarter-mile short and impacts a house. An elderly couple sleeping in an adjacent room is miraculously unharmed. (~unknown) A Mooney Ranger M20C departs on a weekend flight from near Albuquerque, but turns around due to weather near Pagosa Springs. The pilot/owner fails to report to work, and a missing aircraft report is filed on Monday evening. The wreckage and bodies are located on Wednesday morning, with the 55-yr-old male pilot and a 42-yr-old female passenger. [article]

(~8:30PM) A Cessna Skyhawk commonly used by Blue Dot Aviation departs the Lakefront Airport [KNEW] on a training flight. The pilot quickly calls back to return but offers no explanation and declares no emergency. The aircraft crashes into Lake Pontchartrain. A search begins but is soon called off due to poor conditions. The search resumes early the next morning, but the male instructor and male student remain missing. [article]

(~6:00PM) Intense cold and gusting winds are blowing through in a remote area east of Albuquerque, when a call is made about an apparent brush fire near Highway 285. A second call, twenty minutes later, reports a plane may be down. It is discovered that a Mooney MO20 crashed and was destroyed by fire, killing the one man on board. [article]

(~unknown) A Cessna 140 departs the Frazier Lake airport near Hollister, intending a day trip to Watsonville [KWVI]. It never returns and a search is initiated hours after sunset. The 1950 aircraft is found the next morning, crashed near Pacheco Pass, with both occupants (one male and one female) deceased. [article]

(~unknown) A Cessna 172 Skyhawk crashes into forested hills, killing the flight instructor and injuring the student. The Cessna was based out of Frederick, MD [KFDK], and was reportedly flying to Martinsburg, WV [KMRB]. Wreckage reportedly was found at 7AM the next morning, in two separate locations. (~5:50PM) A Cirrus SR22 registered with Orthopedic Aviation Services LLC disappears ile descending through 5,000-ft during an approach, on a flight from Washington Municipal Airport, in Iowa. It explodes on impact and all three are killed. Response units dealt with sleet conditions. [article]

(~10:51PM) The Scaled Composites Model 339 ‘SpaceShipTwo’, a suborbital rocket plane being developed fro Virgin Galactic to someday carry paying passengers for short flights into space, crashed during a test flight. It had been dropped from its carrier plane and evidently exploded when the rocket was ignited. A new fuel system for the rocket was being assessed. Reports indicate one pilot survived after being ejected; the other pilot was killed. [article]

(~9:48AM) A Beech Kingair 200 loses power in one engine on takeoff, then turns sharply and crashes through the roof of a flight simulator. The pilot was a 56-yr-old male former controller, hired to do a ferry flight to Mena, AR. The other victims were in the flight simulator. [article]

(~3:47PM) A Cirrus SR22 returning from a morning flight to Tennessee enters a midfield left downwind for Runway 30 while three helicopters are using the flight training pattern below. The pilot advises the tower controller he has two of the helicopters in sight. Seconds later, a third helicopter collides with the Cirrus, and both aircraft plummet to the ground. The helicopter impacts next to storage units, killing three. The Cirrus pilot and passenger are injured, likely saved by the parachute system that deployed for their aircraft. [article]

(~5:37PM) A light-sport aircraft Hawk Arrow II crashes into a tree near the Country Air Estates Airport [1AR9]. The aircraft was left hanging in a tree, and the 75-yr-old pilot fell 30-feet to the ground when trying to get out. He was found conscious and told rescuers he had lost power while turning in the pattern. He died en route to the hospital. [article]

(~10:20AM) A pilot in a kitbuilt Sonex is killed when he crashes into a picnic area just after taking off. Fishermen reported hearing the engine sputter before the crash, just north of Big Bear Lake. [article]

(~3:10PM) After a return flight from New Orleans, a pilot refuels his Beech 36 Bonanza, then begins taxiing to his hangar. A wheel becomes stuck in soft soil next to the taxiway. In an apparent attempt to free the stuck aircraft, power is added and the Bonanza then travels 300-feet and strikes a golf cart and a fence. The pilot, who had served as a state senator from 1976 to 1980, died on the way to the hospital. [article]

(~noon) A couple depart from their air park home in Port St. Lucie, FL, to attend an air show in Rome, GA. Flying with a group, they take off after a stop in Cordele, GA. The Aeronca Champion climbs to approximately 300-feet then plunges into the ground, killing the pilot, a retired air traffic controller. His wife, a retired flight attendant, is hospitalized with serious injuries. [article]

(~11:30AM) An Aero Commander 112 takes off from Lee Gilmer Memorial Airport [KGVL] but experiences an engine problem. The pilot turns back to the airport, but the aircraft impacts a power pole and nose-dives to the ground, then burns. A witness bravely pulls a man from the left seat, but the fire advances before he can return to rescue the man in the right seat. [article]

(~3:21PM) A Hawker 800XP twin-jet reportedly operated by the Monfort Brothers (Conagra meat packers, Colorado Rockies owners) departs Centennial Airport [KAPA], heading for Latrobe, PA [KLBE]. Roughly 100-miles to the east, after reaching 23,000-ft altitude and topping 400-knots airspeed, the pilot begins a descent and nearly four hours of delay patterns to burn off fuel prior to a no-gear landing. The gear-up landing at [KDEN] was successful, as nobody was injured. [article]

(~11:00PM) A Piper Malibu PA46 returning from a trip to Ankeny, IA crashes while executing a second attempted landing. The pilot was the Libertarian candidate for U.S. Senate and a local ER physician. Weather had deteriorated and the latest METAR was: 10:43pm CDT, temp 55, dew 55, winds N at 13, visibility 5.00 -RA BR, ceiling BKN002 OVC007, altimeter 29.46. [article]

(~10:40PM) A Beechcraft Baron departs on a return flight to Lawrence, KS. Witnesses see and hear the aircraft at low altitude, heading southwest, shortly before it crashes in a residential area. No homes are hit. The three on board, all doctors (including the pilot), are killed. Weather was marginal, and may have been a factor: METAR shows 6-miles visibility with mist and a 1,000′ overcast layer at 10:38PM, which degraded to a 900-ft overcast layer at 10:53PM. [article]

(~8:00AM) A kitbuilt Savannah collides midair with a Cessna 170. Both aircraft were reportedly arriving to land on a dry lakebed where a caterer was preparing meals for a fly-in breakfast. The two aircraft became entangled midair and crashed together into the terrain. [article]

Baton Rouge, LA

(~4:00PM) A Focker-Wulf 190-A8 landing on Runway 13 rolls into the EMAS area and is abruptly stopped by the crushable concrete. Due to the nose-heavy weight on this WWII German fighter, the deceleration causes it to promptly pitch downward and settle on the nose and propeller. The aircraft is reportedly one of only seven in existence, and had recently completed years of local restoration work. [article]

(~4:30PM) A fire breaks out near the Foresta community area, along the western border of Yosemite National Park. Less than two hours later, a Marsh S-2T Turbo Tracker flown by a California Department of Forestry pilot is doing aerial retardant drops when it crashes into the canyon wall, above the Merced River and Highway 140. The tanker is said to be one of 23 operated by the state, and is based in Hollister.[article]

(~1:56AM) A Bell 206 helicopter operated by Air Evac Lifeteam flies a shooting victim in from Waurika, OK but crashes one block from the destination helipad. (~afternoon) A Calidus gyrocopter crashes shortly after taking off from the Rostraver Airport in Westmoreland County. The 62-yr-old male pilot fails to show up for an evening meeting and a search is begun. The search is suspended in the early morning, then resumed with daylight.
